We are a fully integrated Public Health & Wellness Center providing comprehensive medical, behavioral, and community-based care to individuals and families across Louisville. We deliver whole-person healthcare designed to remove barriers, improve outcomes, and support clients at every stage of life.
Our clinic brings together primary care providers, behavioral health specialists, substance use treatment professionals, nurses, and Community Health Workers—all working as one coordinated team.
